Built-in QA: How our EHCP ‘front sheets’ help LAs with quality assurance

This front sheet accompanies every draft EHCP we write. It acts as a built-in quality assurance (QA) checklist for the case officer, clearly flagging any potential issues, gaps or conflicts in the draft that need to be reviewed before a plan is issued or presented at panel.

Conflicting professional advice
“There is a conflict in the advice: The EP advice dated April 2025 states that Marty’s gross motor skills are a personal area of strength. However, the school advice dated January 2025 states that Marty’s gross motor skills are noticeably delayed. Gross motor skills have been informed by the EP advice in this draft for now, but will need to be checked.” -
Missing outcomes for identified needs
“The following identified area of need and recommended provision should have a matching outcome: sensory processing. A placeholder outcome has been included, but please check and amend as necessary.” -
Gaps in information provided
“No information was provided for inclusion in Section A regarding what Alfie finds difficult and how he should be involved in decision making.” -
Unconfirmed service involvement
“Educational advice indicates that Sally is known to Speech and Language Therapy, but no advice has been received from this service. Please check if service is involved/report available.” -
Missing Golden Thread linkage
“The following area of need should have a matching outcome and provision: Social communication needs.”
Amending an EHCP is often an even more complex task than writing a new plan, particularly if paperwork reveals inconsistencies or is not comprehensive enough. Here are some example front sheet comments that could be seen on an amended plan:

New diagnoses not supported by evidence
“Section 4 of the AR notes diagnoses of autistic spectrum disorder (ASD) and global developmental delay. However, there is no reference to this in the previous plan or any new medical report confirming subsequent diagnoses. Please check and amend as appropriate.” -
Safeguarding and social care oversight
“The previous plan notes that Oliver is a Child in Need. There is no reference to this in the AR and no update from social care as to whether this is still the case. Please check and amend as appropriate.” -
Missing or unclear professional input
“The AR report mentions an Occupational Therapist, but no Occupational Therapy advice was submitted to confirm whether input is ongoing. Please check.” -
Outdated medical information
“No up-to-date medical advice was submitted to confirm whether information in Sections C and G of the previous plan remains relevant. The information has been carried across but please check and amend as necessary.”
Supporting LA case officers
